Dear Parent/Carer
COVID-19 Vaccination – Online Consent Form
The School Nursing Service are returning to school to offer children aged 12-15 years either their 1st or 2nd dose of COVID-19 vaccination. Sessions will take place from January – March. If your child misses the vaccine on the day and you have consented the team will return to catch up where possible.
To consent to your child receiving either their 1st or 2nd dose of the vaccination click or search the link below (we will not vaccinate any children on the day without your consent):

YOU NEED TO COMPLETE THIS BY 26th JANUARY FOR YOUR CHILD TO BE VACCINATED IN SCHOOL

Please note:
- If your child has tested positive within the last 12 weeks (91 days), before the date of vaccination, they will not be eligible on the day.
- Children are only eligible for their 2nd dose 12 weeks (91 days) after their first dose – we will check this before administering a vaccine to your child.
- You can also take your child to a community drop-in session if you prefer. This website is regularly updated with dates of clinics available www.northeastlincolnshireccg.nhs.uk/covid-19/vaccineclinics.
- You can also book via the national COVID vaccine booking system.
